Wellness Coach Certification: Options, Requirements and Costs
Contents
Short answer: There is no single "wellness coach certification." Most coaches earn a certificate from a training program, and some go on to the national board credential, NBC-HWC. [S04] [S05] To sit for the board exam, you need to finish an NBHWC-approved training program, log 50 coaching sessions, and hold an associate's degree, 60 college credits or 4,000 hours of work experience. [S01] Exam fees total $500 ($100 application plus $400 exam). [S02] Other organizations offer separate exams; ACE, for example, says its health coach certification is accredited by the NCCA. [S08]
This guide is the starting point for our wellness coach certification pages. It explains the main types of credentials, what each requires, what it costs, and where to go next.
Three kinds of "certified"
When a program says you'll be "certified," check who grants the credential and what it covers. Our guide to certification vs. board certification goes deeper, but here's the short version:
Credential type | Who grants it | Example |
|---|---|---|
Program certificate | The school or training company | A certificate of completion from a coach training program |
National board certification | National Board for Health & Wellness Coaching (NBHWC) | NBC-HWC, after an approved program, coaching log and exam [S01] [S05] |
Other accredited certification | A certifying organization whose exam is accredited by a body like the NCCA | ACE Certified Health Coach [S08] |
A program being "approved" by NBHWC means it met NBHWC's educational standards and that its graduates can apply for the board exam. [S04] It doesn't mean NBHWC endorses all of the program's content or business practices, and NBHWC doesn't offer training or recommend specific programs. [S04]
The NBC-HWC board credential
The NBC-HWC (National Board Certified Health & Wellness Coach) is awarded by NBHWC. [S05] The exam was developed with the National Board of Medical Examiners (NBME), which collaborates with NBHWC to create and manage it. [S05]
Step 1: Finish an approved training program
You must complete an NBHWC Approved Training Program before applying. NBHWC lists more than 138 approved programs, ranging from certificate to master's degree programs, including some offered online. [S01] Training covers five content areas: coaching presence and relationships; theories and models of behavior change; skills, tools and strategies; ethics and professional practice; and health and wellness. [S01]
It's your responsibility to confirm a program's approval status before you enroll. [S04] Our page on NBHWC-approved programs compares researched paths, and our guide to online health coach training explains how online formats differ.
Step 2: Log 50 coaching sessions
After you pass a practical skills assessment in your program, you must complete 50 coaching sessions. [S01] NBHWC's coaching log rules say each session must: [S03]
- Last at least 20 minutes
- Be at least 75% coaching rather than education
- Not be with friends, family or current classmates
- Take place in person, by phone or by live video (text-based sessions don't count)
Sessions can be paid or free, and a group session counts as one. [S03]
Step 3: Meet the education or work requirement
You need an associate's degree or higher in any field from an accredited college. [S01] If you haven't finished your degree but have 60 college credits, you can submit an attestation form. [S01] If you don't have a degree, 4,000 hours of work experience in any field can substitute. [S01] See our guide to health coach certification without a degree for more.
Step 4: Apply and take the exam
You apply during a specific exam window, and applications can take up to 30 days to process. [S01] Exams are given through Prometric testing centers. [S01] NBHWC lists three 2026 exam periods (spring, summer and fall), with a fall refund deadline of November 20, 2026. [S02]
What certification costs
Cost | Amount | Source |
|---|---|---|
NBHWC application fee | $100, non-refundable | [S02] |
NBHWC exam fee | $400 | [S02] |
Approved training program | Varies by program | [S04] |
Example scholarship (Duke) | $500 to $2,500 toward registration | [S09] |
NBHWC refunds the exam fee if you turn out to be ineligible or cancel by the refund deadline. It doesn't cover prep or travel costs. [S02] NBHWC says you may be able to have exam fees reimbursed through the GI Bill. [S01] Training programs set their own policies. Duke, for example, says its program is not a GI Bill-approved training provider. [S09]
Training is usually the biggest cost. Our health coach certification cost guide breaks down tuition and extras.
NCCA-accredited certifications
The National Commission for Certifying Agencies (NCCA), part of the Institute for Credentialing Excellence, accredits individual certification programs that assess professional competency, under its 2021 standards. [S07] Accreditation is renewed on a five-year cycle, and NCCA publishes a directory of accredited programs. [S07]
The American Council on Exercise (ACE) says its Health Coach Certification is NCCA-accredited. [S08] According to ACE:
- You must be at least 18, have a high school diploma or equivalent, and hold current CPR/AED certification that includes a live skills check. [S08]
- Most people study for three to six months. [S08]
- The passing score is 500 out of 800. [S08]
- You renew every two years by completing 20 hours of continuing education. [S08]
ACE certification and NBC-HWC are separate credentials with separate requirements. [S01] [S08]
How programs describe their results
Programs often share their own numbers. Wellcoaches, for example, says 1,500 of its graduates have taken the national exam with a 94% pass rate, and that its CEO co-founded NBHWC. [S10] These are the program's own figures, not independent data. Wellcoaches also cautions students not to rush the process. [S10]
What a certified coach can and can't do
Certification doesn't widen a coach's scope. NBHWC says that on their own, health and wellness coaches do not diagnose, interpret medical data, prescribe, recommend supplements, create meal plans, prescribe exercise or provide psychological therapy. [S06] Coaches can support treatment plans when working under a licensed professional. [S06] Our guide to what a health coach does covers the role in detail.
Certification and Medicare
On July 14, 2026, CMS released its proposed 2027 Physician Fee Schedule. [S11] According to the American College of Physicians' summary, CMS proposes paying for three health and well-being coaching codes delivered by "appropriately trained and certified" staff under a billing practitioner's supervision, and identifies several qualifying certification standards. [S12]
As of October 30, 2026, the rule was still a proposal. [S11] NBHWC says the comment period has closed and expects the rule to be finalized between October and November 2026. [S13] For what this could mean for pay, see our health coach salary guide.
How to choose your path
- Decide which credential you want. If you want NBC-HWC, start with NBHWC's approved program list. [S04]
- Check eligibility early. Confirm you meet the degree, credit or work-experience rule. [S01]
- Plan time for the coaching log. The 50 sessions come after your skills assessment, not during early coursework. [S01] [S03]
- Match the exam window. Applications close at set times, and registrations can't be transferred. [S01]
- Compare full costs. Add tuition, required extras and the $500 in exam fees. [S02]

Reader questions
- What is the most recognized wellness coach certification?
- There is no single official certification. The national board credential is NBC-HWC, awarded by the National Board for Health & Wellness Coaching after an approved training program, 50 coaching sessions and a board exam created with the National Board of Medical Examiners. Other certifying bodies, such as ACE, offer their own exams.
- How much does NBHWC board certification cost?
- NBHWC charges a $100 non-refundable application fee and a $400 exam fee. That is on top of the cost of an approved training program, which you must complete first.
- Can I get certified as a wellness coach without a degree?
- Yes. For the NBHWC board exam, you need an associate's degree or higher in any field, or 60 college credits, or 4,000 hours of work experience in any field. ACE requires a high school diploma or equivalent for its health coach exam.
- Can I become board certified with an online program?
- Yes, if the program is NBHWC approved. NBHWC lists more than 138 approved programs, including programs offered online. Your 50 coaching sessions can be in person, by phone or by live video, but not by text.
- Do I need certification to bill Medicare for coaching?
- Not yet, because there is no national Medicare payment for coaching. CMS has proposed paying for coaching provided by appropriately trained and certified staff under a practitioner's supervision starting in 2027. As of October 30, 2026, the rule was still a proposal.
